WORKING AGREEMENT

For work with institutions and organisations

 

1. My practice is carried out in accordance with BADTH, SESAME & HPC codes of Professional Conduct (copies enclosed).   

2. I undertake to provide documentary proof of my qualification(s), insurance and HPC membership.

3. I shall use my clinical judgement to determine the appropriateness of any referrals to the Group and to determine the most appropriate interventions to use with clients. Any major changes to the manner in which I offer this service will be discussed in full with the relevant member of staff – in most cases this discussion will be with the manager.

4. I shall invoice monthly using the following sessional formula:

 10:00    Arrive & set up room, feed in from Centre Staff

10:30    Group Dramatherapy session

11:30    Feedback to centre staff and clear away room

12:00    Depart 

5. My hourly rate is £40 per contact hour and so this will work cost £80 per weekly session.  In this case my fee includes the following items:

My fee includes a contribution towards Clinical Supervision costs (£40 per hour), Personal Therapy costs (£40 per hour), Professional Membership fees, Public Liability Insurance & Professional Indemnity Insurance, and Continuing Professional Development training.  In addition I will invoice at my standard hourly rate for attandence at Team Meetings, Client Reviews and for time spent writing Client Reports.

7. My hourly rate for 2012 is £40 per hour.  I anticipate that this rate will be reviewed annually and that there will be an increase in line with the cost of living in March 2013. 

8. In the event of my being unable to attend session due to illness or intractable transport difficulties, I will inform you by telephone as soon as possible.  I require at least two weeks notice for cancellation of sessions, otherwise I will invoice for the sessional charge. 

9. Please confirm to me in writing that these terms form part of our working agreement and that they are acceptable to you.
